A 2004 randomized, double-blind, placebo-controlled trial of 60 adults with influenza-like symptoms (started within 48 hours) reported that those taking elderberry extract had symptoms relieved an average of 4 days earlier than the placebo group and used significantly less rescue medication. 2

A 2019 meta-analysis of randomized controlled trials (including the Sambucol® studies) concluded that black elderberry supplementation substantially reduces upper respiratory symptoms, with a large overall effect size. The benefit appeared strongest for influenza-related symptoms. 3
